Dear Parents and Patients,

Thank you for allowing our office to care for your child or children. To better serve everyone, we have had to devise a new policy.

Due to an unacceptably high rate of late cancellations and no-shows, the following policy is being instituted:

Cancellations without a 24 hours or 1 business day notice and failure to show for scheduled appointments will result in a $50.00 fee Per Patient.

You will be asked to sign an acknowledgement and acceptance of the above policy at your child or children's next appointment. Thank you for your understanding.
